Please submit ten (10) paper copies, and a PDF file*, of this application, including any product literature such as brochures, test data, and Materials Safety Data Sheets, etc. to:

Upon receipt, MSD will assign a staff member to review the submitted data prior to distribution to District´s Materials Committee. You may be contacted by the assigned reviewer(s) for additional information.
Upon receipt of the “complete” application and supporting documentation, this information will be reviewed by the Materials Committee at its next regularly scheduled meeting. These meetings are scheduled to occur on the third Tuesday of the months of January, April, July, and October. Complete applications must be received at least 45 days prior to the meeting date to ensure inclusion on the meeting date agenda.
Once the Committee has completed its review the applicant will be notified in writing as to the status of District´s evaluation. The applicant may withdraw the product from the evaluation process at any time by submitting a written request to the above e-mail address.
As part of the product review the District may require the submission of additional testing information or the installation of the product on a trial basis. Refusal of the applicant to comply with District review requirements will result in the rejection of the product.
